Yes this is correct but if governing thickness code is  ASME B 31.3. Otherwise  qualification range will be 8 to 16 mm according to ASME 9.

With impact test thickness range is t/2 to t+6  means (8/2mm =4mm )   to  (8+6=14mm)

 

Means with Impact test thk range is 4m to 14mm

Without impact test thk range is 4.8mm to 16mm is currect

Yes,

I think you are refring to same supporting PQR only for the both thickness ranges.

 

For E.g, If you have qualified a PQR with a thk of 8 mm with impact test.

 

Your qualification ranges in the WPS will be

 

8 to 16 mm (WIth Impact test)

5 to 16 mm (Without Impact test)

 

You can write both the reading in the WPS.
